Allegiance Labs : Collection Sites

Throughout the nation, we have collection sites where we can obtain specimen and send it back to our independent medical lab for analysis. We provide employers with a Chain of Custody Form that specifies what type of test(s) to run and the company that will be billed for the test(s). A witness signature is provided to confirm the validity of the specimen.
